This problem has been around for years but I have not seen it until a few days ago. This video talks through the solution I found to the problem (one possible solution). Hopefully it might help you either solve the problem or maybe just look in the right direction. I have no doubt that this could be caused by multiple things but it's worth trying this as it's so easy to do and it did work for me.I tried some of the other solutions out there but they don't work. It seems that this one will remain a bit of a mystery and plague iTunes users for sometime yes.
